In order to make the initial visit go quicker, please print out the appropriate forms and fill in all the necessary information that applies to you. This will allow us to pre-register you in our computer system. Bring these forms with you to your appointment.
Otherwise, please plan on arriving 15 minutes early to your first appointment in order to fill out the appropriate paper work.
